fix: hardcoded '/' in normalized options.fs, fixes #412, #408

Changes including:
- will not call fs.resolve when normalizing `fs`
- removed hardcoded `/`
- mandatory `fs.dirname` for relative reference
- mandatory `fs.sep`, defaults to '/'
